To be completed by a school counselor, principal, teacher, community contact, employer, clergy, etc. Please provide all information on this recommendation form. If preferred you may also submit your recommendation on your own letterhead with all information requested on this form, to [email protected]. Please include student’s name on subject line. You may also submit questions to scholarship committee member Karlyn Geis at the same gmail address.
